Building a system to
prevent homelessness

Prevention starts with data and coordination.

We use predictive tools to spot risk early and offer help, support partners across the Continuum of Care for the homeless and strengthen the system so people in crisis get help fast, stay housed or get housed.

Cincinnati is proving it can be done.

Housing Stability

Uses predictive data to identify households most at risk and intervenes before a crisis begins.

Partners provide tailored support and resources to keep people in their homes and reduce the need for more costly emergency services.

Shelter Diversion

Helps people at imminent risk of entering a shelter or being on the street find housing.

Over the last five years, 95% of participants remained housed one year after exiting the program, showing how early intervention keeps families stable.

Shelter & Outreach Services

Connects people in unsafe or unsheltered situations to help fast.

Through the CAP Helpline, the Street Reach tool, and our family outreach program, partners identify people in crisis and connect them to shelter, safety, and the support needed to regain stability.

System Coordination

Convenes a coordinated community effort across Cincinnati’s homelessness service system.

As 1 of only 14 organizations nationwide named a HUD Unified Funding Agency, we direct funding, resources, and support to nonprofit providers to promote the goal of ending homelessness.

Coordination of Resources

Ensures federal, state, and local funding is used as efficiently as possible.

One of our roles as a HUD Unified Funding Agency is to establish the most effective and targeted pathways for funding and other resources, streamlining Cincinnati’s community efforts to prevent and end homelessness.
